A ledger row that said the wrong thing
Full backend CRUD and a typed API client existed for a feature that no component anywhere called: endpoints built, shipped and unreachable.
Built, and nothing used it
Shipped, typechecked, dependency-injected, unreachable. Nothing ever called it.
Went to re-verify letting a organisation admin set which documents a program requires. Went looking for the button and never found one, in either admin surface. Backend: full CRUD, four working endpoints. Frontend API client: all four methods, fully typed. Components calling them: none, anywhere.
It had been sitting in the ledger as "not yet re-tested this pass, prior evidence stands," which reads as ordinary backlog. The real state was that there was never anything to click. A backend-plus-client pair that compiles cleanly and has API-level coverage can still be completely unreachable, and nothing about "the code looks done" will tell you — only going to look for the button will.
